New Clients

The following forms are designed to help us work together more easily. This information will help me get to know you and understand your goals so I can specialize the workout we do with routines designed just for you.

Once you have decided to start, there is a training agreement that you will want to review and get back to me before our first session.

Howse Client Intake Form

If you are a first-time client, please print this form, complete it, and bring it along to our first session. Please answer the questions to the best of your ability, but don't get too caught up on anything you don't know the answers to. We will go over it again together.

3 day dietary record

An up-to-date eating diary will help keep you on the right track and provide a good update on your nutrition practices and areas to work on when required. Getting you into the habit of keeping an eating diary or at least really thinking about what you're eating will inherently help you stick to a better nutrition programs.

Howse Fitness Training Agreement 2022

Once you've decided to start your training, you will need to review and sign this agreement. It covers most basic questions as well as pricing, cancellations, and rescheduling. Please review, sign, and return to me via HowseFoundation@yahoo.com or at your first in person appt
